Job Description

Role Summary: Registered Nurse - Triage & Patient Care

The RN assesses patient concerns via telephone triage, manages messages and test results, provides education, coordinates care, and supports clinical staff. Requires strong assessment, communication, and critical thinking skills. Must have an active Minnesota RN license, CPR certification, and at least two years of clinical experience, including one year of triage.

Essential Duties & Responsibilities:

  1. Under the general supervision of the Providers, performs professional nursing functions to assess the severity of the patient's health concerns using established protocols, standing orders and guidelines to assess and manage patient health concerns and symptoms via a telephone interaction. These telephone interactions require assessment, patient education and may require crisis intervention.
  2. Responds to and prioritizes incoming patient messages and test results.
  3. Responds to patient/family questions and appropriately provides reassurance while responding to patient/family concerns.
  4. Obtains appropriate input and direction from the Provider as needed to determine the best clinical course of action and recommendations for specific patient health care needs.
  5. Accurately documents symptoms/complaints, nursing assessment, advice provided and patient/caller response. Follows policies, procedures, and protocols to ensure consistency and departmental effectiveness as well as improve health care outcomes of patients/callers and their access to appropriate health care.
  6. Manage prescription refills per protocol.
  7. Participates in the care coordination team process.
  8. Provides patient education, e.g., chronic disease preventive, lactation, diagnostic procedures, medications, nutrition, etc.
  9. Available to other office staff to answer questions and provide assistance as appropriate.
  10. Assists in orienting and training new staff.
  11. Seeks opportunities to improve or enhance nursing and leadership skills.
  12. Completes other duties as assigned.

Qualifications and Requirements: Bachelor's, Associate or accredited program graduate; board-certified with a current, active and unrestricted Registered Nurse (RN) license in Minnesota. Must be CPR certified and maintain compliance with all state and regulatory requirements, including continuing education and timely license renewal.

Experience: Minimum of 2 years work experience as a RN in a clinical or related setting and ideally triage experience of at least 1 year.

  • Demonstrates clinical competence.
  • Capacity to function independently within the license scope of practice.
  • Must have the ability to handle stressful emergency situations calmly and effectively.
  • Works effectively with others to accomplish objectives and goals. Offers assistance to others in need.
  • General computer skills required. Experience with EHR systems.

Requirements: A current and active Registered Nurse (RN) license in Minnesota is required, with ongoing compliance with all state and regulatory requirements for licensure maintenance, including continuing education and timely renewal.

Physical / Mental Demands: Requires sitting and standing associated with a normal office environment. Must be able to use appropriate body mechanics techniques when making necessary patient transfers and helping patients with walking, dressing etc. Must be able to lift up to 30 pounds.

Environmental / Working Conditions: Office, exam/procedure rooms setting. Exposure to situations common to the clinic setting, including communicable diseases, bodily fluids, and sharp instruments. Occasional evening or weekend work.

Hours:

  • Monday 7:30 am - 12:00 pm
  • Wednesday 7:30 am - 7:00 pm
  • Thursday 7:30 am - 5:00 pm
  • Friday 7:30 am - 5:00 pm
  • Saturday - rotating shifts 8:30 am - 12:00 pm
