Job Description

Family Service Rochester is seeking a full-time Social Worker in our Guiding Partners to Solutions (GPS) team, focusing on Mental Health and Homelessness. As a social worker specializing in mental health and homelessness, you will provide comprehensive case management services to individuals experiencing homelessness and mental health challenges. You will engage with individuals, assist them in obtaining stable housing and connect them with supportive community resources to enhance their overall well-being and quality of life. You will do this by:  

  • Building trusting relationships with individuals through consistent and compassionate outreach.  
  • Conducting comprehensive assessments to identify individuals’ needs, strengths, and goals. 
  • Assisting individuals in navigating housing systems and securing stable housing.  
  • Collaborating and partnering with housing providers, landlords, and other stakeholders to facilitate housing placements.  
  • Connecting individuals with mental health services, substance use treatment, medical care, and other supportive services. 
  • Developing individualized service plans in collaboration with other service providers.  
  • Advocating for individuals’ needs within the community and with service providers.  
  • Providing crisis intervention and support as needed.  

Eligible candidates will hold a bachelor's degree in social work, psychology, or a related field (master's degree preferred) and have a minimum of 2 years of experience in case management, preferably with unhoused populations and/or individuals with severe and persistent mental illness. Preference will be given to those with a Minnesota Social Work License or license eligibility. 

Our ideal candidate works effectively with diverse populations, has excellent communication skills, is comfortable working independently, as well as working closely with a multidisciplinary team, has experience with crisis intervention, values collaboration with community partners and has a working knowledge of trauma and how mental health and chemical health can impede resource access. Must possess computer literacy. Bilingual abilities are a plus.
