ASCEND is seeking a Registered Nurse (RN) to work in various program sites within our agency and community (local travel required). Must have current licensure as a Registered Nurse by New York State. Continued employment is dependent upon retention of licensure.

Must have 2-3 years of clinical experience, preferably in mental health, along with a valid driver’s license.

Responsibilities include, but are not limited to:

• Direct oversight for the Agency LPN positions and responsibilities.
• Directs client care plan activities.
• Act as the Tobacco Cessation Specialist for the Agency.
• Lead as medical liaison with external medical providers for client care.
• Lead and participate as necessary in the delivery and evaluation of care and assists staff in ensuring the health, comfort, and safety of clients and help to maintain the integrity of the client care.
• Coordinate health education for staff and clients as necessary.
• Coordinate and operate the Clinic’s Psychotropic Medication Injection service for identified clients as needed.
• Coordinate and operate the Health Monitoring program at Caleo.
• Relies on experience and judgment and a certain degree of creativity and latitude to plan and accomplish goals.
• Performs a variety of tasks including administrative duties such as completing medical forms, reports, evaluations, charting, etc., as necessary, and ensures the accuracy and completeness of clinical documentation.
• Coordinate and assist Caleo Nurse Practitioners with prior authorizations, IStops, and phone calls to clients regarding medication issues.
• Coordination of the Clozaril program at Caleo Counseling.

Pay: $60,000.00 - $74,000.00 per year
