Blize Healthcare is looking for a Medical Social Worker to join our team.

Job Description:

• Responsible for facilitating patients and families in the identification and management of bio-psycho-social, spiritual, financial, development, and cultural issues in the face of acute, chronic, or terminal illness to keep them safe and adequately cared for at home.
• Reviewing new admissions for any special needs or issues that need to be addressed.
• Assisting with patient discharging from a hospital, clinic, or nursing home setting, to return home.
• Coordinating the care of patients with complex needs that require multiple services over time.
• Developing a plan of care for each assigned patient.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
• Assessing patient needs, plans, evaluates, and modifies care to meet the goals of social work interventions.

Requirements Include:

• Master’s degree in social work (Required);
• Current CPR certification (Required);
• Driver's License (Required).
