Hospice Registered Nurse (RN Case Manager)

Salary and Benefits

• Salary Range: $86,000 – $91,000 (based on hospice experience)

Responsibilities

• Full-time position; Monday to Friday
• Based out of the Marietta, GA office
• Manage a dedicated caseload of home-based hospice patients
• Coordinate care with a comprehensive interdisciplinary team
• Perform patient assessments and develop individualized care plans
• Provide clinical oversight and symptom management
• Educate patients and families on the hospice philosophy and support services
• Attend IDG meetings and contribute to team-based care planning
• Supervise LPNs and hospice aides when appropriate
• Maintain accurate and timely documentation in HCHB EMR system

Requirements

• Active RN license in the State of Georgia (unencumbered)
• Hospice experience is required (Home Health, ICU, and ER experience alone will not be considered)
• Current CPR certification
• Valid driver's license and reliable, insured vehicle
• Strong skills in clinical documentation, communication, and patient assessment
• EMR experience preferred (HCHB specifically)
• Must reside locally or be willing to relocate (due to traffic and territory coverage)
