The Registered Nurse (RN) works next to the Clinical Manager and/or Executive Director to initiate the start of care (Oasis) and coordinate the development and implementation of care plans for their clients that drive the physician ordered clients' home care services in accordance with current standards and regulations of the state and agency. While adhering to confidentiality standards and professional boundaries, at all times.
Schedule: Monday - Friday visits needed, 22.5 points (30 hours) w/weekend on-call rotation
Territory: Newton area and surround.
Essential Job Functions:
• Schedule Admissions/Start of Care visits with client/ client family members
• Perform client intake assessments
• Utilizing O complete and document the client start of care
• Submit required documentation for prior authorization of client services as needed based on client's pay sources
• Document any verbal orders for the Admission/Start of Care through timely communication with the client’s physician and other caregivers
• Support agency audits
• Support other visits: Skilled Nursing Visits, Resumption of Care, Recertifications, Discharges, and Supervisions as needed
• Provide care in patients home using a variety of skills such as: phone triage, patient education, observation/assessment, wound care, infusions, catheter care, PICC line dressing changes etc.

Requirements:
• An active RN License in the state of application
• Valid CPR
Preferred:
• Medicare Skilled Nursing experience
• Basic understanding of Oasis
• ICD 9/10 knowledge a plus
• 1-year RN experience in a health care setting
