Could do Monday, Wednesday, Friday. *Full-Time Opportunity Available with Mon-Fri dayshift schedule.
Shift : Day Shift  Location: 30-45 mile radius from hospital. Areas could include in Kansas: Wyandotte, Leavenworth, Douglass, Johnson, Miami, and Franklin Counties. In Missouri: Clay, Cass, Jackson, and Platte Counties. The role you'll contribute to:  This position is accountable for assisting the health care professional by providing personal health care services and assistance with activities of daily living to the patient as specifically identified in a written assignment and necessary for the accomplishment of patient care plan goals.  The value you’ll bring to the team:

• Punctual with returning pages and phone calls, with completing documentation, timeliness in arriving at meetings and for patient treatments.
• Maintains a good attendance record, notifies supervisor of absences in a timely manner.
• Establishes effective rapport with patients, their families, and co-workers.
• Respects authority by following the appropriate channels of communication in resolving problems and presenting information.
• Demonstrates flexibility in accepting patient assignments.
• Communicates changes in the patient’s condition with the Case Manager or the Supervisor in a timely manner.
• Provides care with respect for the patient’s privacy and property.
• Demonstrates supportive and positive attitude of Agency’s policy/procedure and practice to the community.
• Protects privileged information.
• Demonstrates Agency values when dealing with other health care providers and community resources.
• Maintains confidentiality of patients and other associates by avoiding inappropriate discussions and gossiping.
• Performs personal care activities and supportive services for patients in their place of residence as specified in a written assignment.
• Follows appropriate safety measures when providing patient care.
• Communicates pertinent information to the PCS, Case Manager and other team members.
• Follows plan established on the HHA assignment sheet.
• Documentation clearly reflects HHA assignment.

Qualifications

The expertise and experiences you’ll need to succeed:

• Kansas Home Health Certification if not actively holding certification, certification must be acquired within 45 days License.

• BCLS obtained through the American Heart Association

• Valid Driver’s License

• Current Car Insurance

• Appropriate State CNA license

• Preferably, have had at least one year’s employment experience as a Nurse’s Aide in an institutional setting. 

• High School Diploma or Equivalent Preferred
