Position Overview:
As a Clinical Pharmacist, you will play a key role in improving patient outcomes by providing comprehensive medication management services. Your expertise will help optimize medication therapies and ensure patients receive the most effective treatments tailored to their individual needs. In this role, you'll deliver Medication Therapy Management (MTM) services, including medication reviews, adherence monitoring, cost-effective interventions, and transitions of care, to enhance the overall healthcare experience for patients.
Key Responsibilities:
Provide Medication Therapy Management (MTM) services, such as Comprehensive Medication Reviews and Disease-Specific Medication Reviews, to assess the appropriateness of prescribed medications and identify opportunities for improving patient health outcomes.
Review patients' medication profiles using evidence-based guidelines to develop recommendations that optimize medication regimens.
Implement cost-effective strategies to manage medication expenses, such as exploring formulary alternatives, pill-splitting options, and identifying unnecessary or duplicate medications.
Evaluate prescribed medications for potential issues such as unnecessary treatments, incorrect doses, ineffective therapies, adverse drug reactions, and adherence challenges.
Create and communicate personalized Medication Action Plans (MAPs) and a Personal Medication List (PML), collaborating closely with healthcare providers to ensure optimal care.
Serve as a resource to patients, healthcare providers, and interdisciplinary teams, offering expert advice on medications and therapy options.
Advocate for MTM services, ensuring patients, their families, and providers understand the benefits of these services in managing health outcomes.
Deliver excellent customer service by providing prompt, professional, and polite responses to patient and provider inquiries.
Follow up with patients and providers to track progress and ensure treatment goals are met.
Contribute to the development and implementation of additional clinical pharmacy services as needed.
Meet productivity and performance goals as set by the manager.
Qualifications:
A Bachelor's degree in Pharmacy or a Doctor of Pharmacy (Pharm D.) from an accredited pharmacy school.
A minimum of 2 years of experience as a clinical pharmacist.
Experience in Medication Therapy Management (MTM) is preferred.
Active, unrestricted pharmacist licensure in your state.
Willingness to obtain pharmacist licensure in additional states if required.
Strong interpersonal and customer service skills with the ability to communicate effectively both in writing and verbally.
Proficiency with IT tools, including MS Office (Excel, Word) and Google G-Suite.
High-speed internet connection (cable broadband or fiber optic service provider).
Physical Demands:
This role requires typical office-based physical activities such as sitting, standing, and occasional light movement.
You will primarily work in an office setting, using a computer and phone for extended periods.
The work environment is secure and fully compliant with HIPAA, ARRA/HITECH, and federal/state regulations.
This position is performed remotely in a home office setting, offering flexibility for your work-life balance.
